OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certificates, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of persons who have been barred from practicing in other states.
The department’s Division of Health Systems Quality Assurance works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (eg, dentists, nurses, counselors). Information about disciplinary action taken against physicians and physician assistants can be found at Washington Medical Commission (WMC) website.. Questions regarding WMC disciplinary action may be directed to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Apply for a health care provider license” in the “How do I do it?” section i Home page of the Department of Health (doh.wa.gov). The site includes information about a health care provider’s license status, their credential expiration and renewal dates, disciplinary actions, and copies of legal documents issued after July 1998. This information is also available by calling 360-236 -4700. Consumers who feel a health care provider has acted unprofessionally are also encouraged to call and report their complaint.
Clark County
In June 2022 the secretary of health suspended the certified nursing assistant license. Rachel Marie Teague (NC60684365). Teague did not respond to an investigator’s request for an explanation of allegations that she stole $90 from a customer.
King County
In June 2022, the secretary of health suspended her registered nursing assistant license Gladwell W. Kagai (NA00200820). Kagai was drunk while tending to a patient.
In June 2022, the Board of Nursing suspended the registered nurse Bryan David Maruhashi (RN60563235) after Maruhashi was convicted of four counts of assault in King County Superior Court.
Pierce County
In June 2022, the Medical Assistant Program permanently suspended the medical assistant Sharon Linda – I’m not afraid (CM60967376) pending further disciplinary action. The Adult Protective Services Department of Health and Social Services determined that Nakazzibwe neglected a vulnerable adult, which prohibits him from being employed in care or having unsupervised access to vulnerable adults.
Spokane County
In June 2022, the secretary of health suspended the certified nursing assistant and medical assistant licenses of Samantha Marie Shivers (NC60781594, MR60861969). Shivvers did not respond to an investigator’s request for an explanation of allegations that she stole items from her employer, including an iPhone.
Yakima County
In July 2022, the health secretary suspended the certified home care aide license Michelle Diane Reyes (HM60647490). Reyes pleaded not guilty to charges related to her admission that she took medication from patients on several occasions at the residential home for the elderly where she worked.
Abroad
Nevada: In June 2022, the Pharmacy Commission finally suspended, pending further disciplinary action, the pharmacist’s license of Aurora Bejto (PH60957232). In July 2021, the Nevada State Board of Pharmacy suspended Bejto’s pharmacist license for two years for diversion of controlled substances.
