A team from the Johns Hopkins University School of Nursing will present more than a decade of work on CAPABLEa program that reduces the impact of functional impairment and disability, increases motivation and self-efficacy, improves emotional health, and reduces hospitalizations and nursing home days. CAPABLE is based on the concept that improving health involves one’s environment, including the home, thereby enabling older adults and their families to change behaviors that are essential to aging well that result in improved health, independence, and safety at home.
Panelists:
Melissa deCardi Hladek, PhD, CRNP, FNP-BC. is an Assistant Professor at the Johns Hopkins University School of Nursing and a researcher-clinician focusing on how stress, self-efficacy, and sociocultural factors influence the biology of aging, chronic disease, and frailty.
Tiffany Riser, MSN, MPH, RN, ANP-C, PhD student and research assistant at Johns Hopkins School of Nursing. She has extensive experience in providing home care in rural and urban communities. Her research focuses on implementing home care interventions within health care systems to promote aging in place.
Sabrina Elias, PhDc, MSN, RN is a doctoral candidate and research coordinator at the Johns Hopkins School of Nursing. She earned her Bachelor and Master of Science in Nursing from the University of São Paulo, Brazil. Sabrina is a public health nurse researcher and former international consultant for the Pan American Health Organization/World Health Organization (PAHO/WHO). She has extensive experience working with countries in Latin America and the Caribbean.
